Stefan Brüns 78782dcd53 Fix static initialization order for UserFormatRegister map
A std::map is in an invalid state when just zero-initialized, and needs
to be initialized by its constructor. As this initilization may be done
after the first call to Register, a crash will typically happen.

To fix this wrap all accesses to the map with a Meyers Singleton. Also
remove the extra Array - most accesses are using the key, and the few
format list iterations all sort the result afterwards anyway.

Fixes #201.

Netgen mesh generator

NETGEN is an automatic 3d tetrahedral mesh generator. It accepts input from constructive solid geometry (CSG) or boundary representation (BRep) from STL file format. The connection to a geometry kernel allows the handling of IGES and STEP files. NETGEN contains modules for mesh optimization and hierarchical mesh refinement. Netgen 6.x supports scripting via a Python interface. Netgen is open source based on the LGPL license. It is available for Unix/Linux, Windows, and OSX.
